5-methyltetrahydrofolate (5-MTHF) is the biologically active form of vitamin B9, also known as folic acid or folate. In conjunction with vitamin B12, 5-MTHF acts as a methyl donor for the conversion of homocysteine to methionine. This process of methyl donation plays a vital role in numerous processes of the body.

5-MTHF, along with pyridoxine and Vitamin B12, plays a critical role in one-carbon metabolism.
They are essential for the functioning of the enzyme methionine synthase which regulates homocysteine concentrations.

Homocysteine accumulation has been found to affect female reproductive functions including reduced cell division, inflammatory cytokine production, altered nitric oxide metabolism, increased oxidative stress, elevated apoptosis and disturbed methylation reactions.

Polymorphism of the MTHFR gene impairs the activity of the MTHFR enzyme which converts folic acid into the biologically active form 5-MTHF.

MTHFR polymorphism thus results in lower bioavailability of folate and Vitamin B12, leading to increased homocysteine concentrations.

Polymorphisms in the MTHFR gene and hyperhomocysteinemia have been found to be associated with increased risk of spontaneous abortion, gestational hypertension and female subfertility.

5-MTHF has been shown to be an adequate alternative to folic acid in reducing total homocysteine concentrations.
